Medication Protocol for Upcoming 8th Grade Washington, D.C. Trip

Prescription Medication Bottles

Mrs. Shaw, the school nurse, will be at AMS to collect medications during school hours (7:30 am - 2:35 pm) on:

  • Monday, April 30th
  • Tuesday, May 1st
  • Wednesday, May 2nd

This is the week before students leave for Washington, D.C. Please plan accordingly.

Please be aware of the following:

  • Both prescription and non-prescription medications must be brought to school with the district form (forms available online or from the AMS clinic).
  • Prescription medication forms must have your child's physician's signature (this can be faxed to your child's physician and faxed back, if needed).
  • All medication must be in the original bottle.
  • No medications will be collected the day of the trip, May 9th.

Thank you for your cooperation with this policy. The safety of your child is of the utmost importance. Please ensure your child has a safe trip by paying close attention to the medication protocol the district follows.

Please contact Mrs. Matters ( smatters@athenscsd.org) or Mrs. Shaw (hshaw@athenscsd.org) if you have any questions.
